Whether you are managing your own healthcare or helping an aging parent, At Your Call offers scheduled support with appointments, care information, provider communication, and agreed next steps. Family updates follow the client’s preferences and appropriate authorization.
These plans also support other adults with coordination needs. Care coordination, private home care, bedside recovery nursing, and IV treatments are arranged separately.

For one enrolled person.
In-home visits and appointment accompaniment are arranged and quoted separately.

The in-home visit counts toward the five-hour allowance; it is not additional to it.
Discuss Personal Nurse Support$1,300/month
For up to 2 enrolled adults in the same household.
For up to two enrolled adults in the same household. The visits count toward the eight-hour allowance, and the allowance is shared by both members, not eight hours per person.

Consultations, appointment preparation, record review, provider communication, client-related documentation, and included visits count toward your monthly service allowance. We provide a time-use summary and discuss additional work before exceeding your allowance. Work is never counted twice.
Additional agreed care coordination is $125 per hour, billed in 15-minute increments. We ask for your authorization before your allowance is exceeded and provide a clear summary of time used. This rate applies only to care coordination—not home care, bedside nursing, or post-surgery visits.
Your written agreement confirms any applicable travel charges before enrollment. Memberships do not include 24/7 access, unlimited coordination or visits, immediate responses, or guaranteed staffing.
