The Senior Concierge Package
Specifically designed for seniors and their families seeking proactive and continuous support, our flagship package ensures you never have to face health challenges alone.
For a monthly retainer of $1,500 (covering up to two members in the same household), this comprehensive package includes:
- 24/7 Concierge Nurse Access: A dedicated source of round-the-clock healthcare guidance and support, in person or over the phone.
- Monthly Health Assessment: A regular check-in to monitor and evaluate your health status, helping identify changes early.
- Convenient Medical Appointments: Access to up to two medical appointments per month within a 20-mile radius.
- In-Depth Consultations: Three monthly consultations (in person or via Zoom) to discuss medical management, access to care, and any concerns.
This package integrates seamlessly with our other specialized services, including Chronic Care Management, Transitional Care after a hospital stay, and dedicated Patient Advocacy, creating a true circle of support tailored to your evolving needs.
