My service also offers Transitional Care Management (TCM) services. In the 30 days following discharge from acute care, such as a hospitalization or inpatient stay, we provide essential support to help patients transition smoothly between care settings. Our services include:
- Medication management and treatment oversight
- Scheduling follow-up appointments with providers and specialists
- Educating patients and/or caregivers about the condition
While TCM encompasses a wide range of temporary services, Healthcare Connect is dedicated to supporting patients post-discharge, preventing medical errors, and reducing the risk of readmission.
